Lost Health Insurance Card or Eligibility Confiemation Document
Insured persons and dependents need to handle their health insurance card or Eligibility Confirmation Document with care because the Salesforce Health Association cannot freeze them even if lost or stolen.
If your health insurance card or Eligibility Confirmation Document is lost or stolen, you must immediately report the loss to the police and complete the lost health insurance card or Eligibility Confirmation Document procedure with the Health Insurance Society.
After December 2, 2024, a health insurance card will no longer be issued, please register your Individual Number Card as a health insurance card or apply to issue an Eligibility Confirmation Document.
Insured persons with optional continued insurance should contact the Salesforce Health Association.
