The Salesforce Health Association pays funeral expenses (family member burial expenses) if an insured person or dependent dies. Please make sure to complete the procedures for the loss of health insurance eligibility.

Death of an Insured Person
The Salesforce Health Association pays a uniform 50,000 yen for the funeral expenses in the event an insured person dies and is buried by family.
If there was no family member present, the person who actually performed the burial will be reimbursed the actual cost within the total amount of 50,000 yen for the burial fee and 150,000 yen for the additional benefit as "burial expenses."
Death of a Dependent
The Salesforce Health Association pays a uniform 50,000 yen for family member burial expenses in the event a dependent dies.
Please enter the required items on the Application Form for Payment of Burial Fees (Expenses)/Surcharge and submit it together with the required attached documents. (Refer to the Application Forms page for submission destinations.)
Please make sure to complete the procedures below.
- Document certifying death (either employer certification, copy of death certificate, copy of burial permit, copy of cremation permit, etc.)
- Return the Health Insurance Card or the Eligibility Confirmation Document if he/she has it (notification by employer)

[Funeral expenses: if the claimant is not a dependent of the Salesforce Health Association]- Documents to verify financial support (documents that include the deceased insured person and the claimant, such as a certificate of residence)
- Receipt for expenses incurred for burial (original copy addressed to the name of the claimant)
- Document certifying death (either employer certification, copy of death certificate, copy of burial permit, copy of cremation permit, etc.)
- Attach the Health Insurance Card or the Eligibility Confirmation Document if the person has it to the Notification of Change of Dependents
- Application Form for Payment of Burial Fees (Expenses)/Additional Benefits
