Influenza Vaccination
Health insurance societies provide subsidies for influenza vaccination.
In order to prevent the onset of infectious diseases and to prevent serious illness in the event of infection, it is effective to receive vaccinations.

- Name of vaccinated person（If more than one person has been vaccinated, the names of all）
- Vaccination date
- cost（unit price）
- proviso Description of "influenza vaccination"
- Seal of a medical institution or person in charge
- Please take separate receipts if more than one person is vaccinated at the same time. If the healthcare provider cannot issue separate receipts, please ask the healthcare provider to include the names of the people who have been vaccinated and a break down of the charges.
If a portion of the vaccination cost is borne by the national or local government (subsidy), the health association will cover the amount after deducting the subsidy amount.
Employees can also be vaccinated at home or at a in-network healthcare provider near the office. The Salesforce Health Association will provide more details in a separate email.
